Arginine With Blood Pressure and Heart Medicines: What to Know

Nitric oxide supplements and several common heart drugs work on the same pathway. At capsule doses the risk is small, but it is the reason to ask first.

Why arginine and heart medicines overlap

Arginine is the raw material for nitric oxide, which relaxes blood vessels by raising cyclic GMP in their smooth muscle[1]. Several common medicines act on exactly this pathway. Nitroglycerin and isosorbide release nitric oxide directly. Sildenafil, tadalafil and vardenafil slow the breakdown of cyclic GMP. Many blood pressure drugs lower pressure by other routes that add to the same end result.

Supplements that raise nitric oxide therefore share, in a much weaker form, some of the effects of these drugs.

How big is the effect?

At gram doses, modest. A meta-analysis of 11 trials found that 4 to 24 g of arginine a day lowered blood pressure by about 5 mmHg systolic and 3 mmHg diastolic[3]. Because nitric oxide synthase is usually well supplied, effects tend to be larger where nitric oxide production is impaired[2]. At the few hundred milligrams in a typical capsule, an effect on blood pressure is unlikely but cannot be ruled out in someone already on several medicines.

Combinations to discuss with your doctor

If you takeConcern with arginine or citrulline
Nitroglycerin, isosorbideAdditive vessel relaxation; nitrates with PDE5 inhibitors are already a dangerous combination
Sildenafil, tadalafil, vardenafilSame downstream pathway; additive blood pressure lowering
Two or more blood pressure drugsSmall additive effect; watch for dizziness on standing
Spironolactone, ACE inhibitors, ARBs, with kidney diseaseArginine can raise potassium at high doses

After a heart attack

The VINTAGE MI trial gave 9 g of arginine a day, or placebo, to 153 people after a first heart attack. Arginine did not improve vascular stiffness or heart function, and six participants on arginine died during the six months compared with none on placebo; the safety board stopped enrollment and the authors recommended against arginine after a heart attack[4]. The dose in memory capsules is far lower, but cardiologists generally advise caution.

Practical advice

Arginine is generally well tolerated in healthy adults, with gut upset the main complaint at high intakes[5]. If you take any heart or blood pressure medicine, show the supplement's label to your doctor or pharmacist before starting, stand up slowly for the first days, and stop if you feel faint. NCCIH recommends telling every clinician about all supplements you use[6].

Quick answers

Can I take arginine with blood pressure medicine?

Ask your doctor first. Arginine lowers blood pressure slightly at gram doses and could add to medication effects.

Is arginine safe with Viagra?

Discuss it with your doctor. Both act on the nitric oxide and cGMP pathway, so the combination could lower blood pressure further.
