Brain Health Habits That Have Real Evidence Behind Them
The memory interventions with the strongest evidence are not sold in bottles. Here they are, with the trials behind them.
Better evidence than any capsule
When people ask what they can do for their memory, the most honest answer is that the interventions with the best evidence are not supplements. They are ordinary, sometimes unglamorous habits, tested in large trials and long-term studies. The 2024 Lancet Commission estimates that addressing 14 modifiable risk factors could prevent or delay around 45% of dementia cases[5].
Move: aerobic exercise
In a randomized trial of 120 older adults, a year of moderate walking three times a week increased the volume of the hippocampus, a brain structure central to memory, by about 2% and improved spatial memory, while the stretching control group's hippocampus shrank slightly[2]. Brisk walking, cycling, swimming and dancing all count.
Combine: the multidomain approach
The FINGER trial in Finland enrolled 1,260 older adults at higher risk of decline. Over two years, those given diet advice, exercise, cognitive training and monitoring of vascular risk factors scored better on a composite cognitive test than those given general health advice[1]. The lesson is that several modest changes together matter more than any single one.
Control blood pressure
In SPRINT MIND, treating systolic blood pressure to under 120 mmHg, compared with under 140, reduced mild cognitive impairment[3]. Midlife hypertension is one of the Lancet Commission's major modifiable risks[5]. Know your numbers and take treatment seriously.
Sleep
Short and broken sleep is one of the most common causes of daytime fog. In mice, sleep increased the clearance of metabolic waste from the brain compared with wakefulness[4]. Regular sleep times, a dark cool bedroom, limiting late alcohol and caffeine, and getting snoring with daytime sleepiness checked for sleep apnea are practical steps.
Hearing, vision, social contact and the rest
- Hearing: untreated hearing loss is among the largest modifiable risks; hearing aids are worth using if recommended.
- Vision: untreated vision loss was added to the Lancet list in 2024.
- Social contact and mental activity: staying engaged with people and learning new things.
- Smoking, alcohol, cholesterol and diabetes: each linked to risk and each modifiable[5].
The NIA's guidance on cognitive health sets out many of the same steps[6].
Quick answers
What is the best thing I can do for my memory?
Regular aerobic exercise has some of the strongest direct evidence, ideally combined with blood pressure control, good sleep and social engagement.
Can lifestyle changes prevent dementia?
They cannot guarantee it, but the Lancet Commission estimates around 45% of cases are linked to modifiable risk factors.
